Here’s how to get started…

1. Call us at 214-553-8033 and schedule your pet’s appointment.
2. Retrieve your pet’s previous medical history. Call your pet’s previous veterinarian, request the release of the medical records and have them faxed to us at 214-553-9148.
3. Print the “New Client Information Form and Coupon” (button found below).
4. Complete the “New Client Information Form” and bring it to your pet’s appointment. When you provide the completed form, you’ll save $5.00 on your first visit!
